Psychological Reports & Letters.
Professional clinical documentation where appropriate to the purpose.

Documentation with a defined clinical purpose
Clinicians may provide appropriate letters and reports on diagnosis, prognosis and occupational matters following assessment. The practice does not offer medico-legal reports.
Some circumstances require a professional letter or clinical report. Appropriate documentation may be available following assessment, subject to the clinician’s judgement and the nature of the request.
- Diagnosis or treatment-related letters where appropriate
- Occupational or recommendation-related reports
- Scope and quotation agreed in advance
- No medico-legal reports
Can I request a report without an assessment?
A clinician will advise what assessment or information is needed before any document can be prepared.
Do you prepare medico-legal reports?
No. The existing practice website explicitly states that medico-legal reports are not provided.
A considered first step.
You do not need to have everything figured out before getting in touch. Tell us what support you are looking for and we can discuss the next step.
